Personal data of clients are processed by Verloskundigenpraktijk Weesp for the purpose of the

WHAT PERSONAL DATA ARE PROCESSED BY VERLOSKUNDIGENPRAKIJK WEESP

Name, first names, initials, title, gender, date of birth, address, zip code, city, telephone number, e-mail address and similar information of the pregnant woman and her partner required for communication. Medical data and other special personal data, including citizen service number, if and to the extent necessary for the performance of the medical treatment agreement and the declaration of costs to the health insurer;

ONLINE REGISTRATION OF PERSONAL DATA
Personal data are registered in the online obstetric registration system Onatal which complies with
security standards set forth in the AVG law. This system can only be accessed by practice-registered midwives and assistants. All of these healthcare providers have a legal duty of confidentiality (professional secrecy). Registration of name and address, date of birth, BSN and health insurance number are digitally processed for the purpose of client file, identity verification and submission of claims to the health insurance company.
E-MAILADRES
E-mail addresses are digitally processed for the purpose of sending: invitations to participate in surveys for the purpose of evaluating the care provided by the midwife’s practice, possibly temporary important information. This is done via secure e-mail. Partner email address is basically not used.
SHARING HEALTH STATUS WITH OTHER HEALTH CARE PROVIDERS
Data processing of clients’ past and present health status, health status of the (future) child, examinations and treatments performed are recorded for the purpose of providing care. This information is shared with other health care providers only when medically necessary and for the purpose of care. The secure systems, Zorgdomein or Zorgmail, are used for communication with other health care providers. The care providers include gynecologists, clinical obstetricians, pediatrician, general practitioner, consultation office, maternity care and the health insurance company regarding claims.
PERINED
Data on pregnancy and birth are provided to thePerinatal Registry for the purpose of national quality improvement and research. You can read information about your privacy at Perinatal Registration on the website www.perined.nl/privacy Perined manages a database of medical data on behalf of health care providers. The mother’s zip code and date of birth are recorded, this makes the person traceable.
PERIDOS
If you are participating in scraings programs PNS, 13 wk and 20 week ultrasound and heel prick, data is exchanged with the national database Peridos for the purpose of carrying out screening and any treatments, and for the purpose of ensuring quality of care. More information about your privacy in screening programs during pregnancy can be found in the leaflets and on the programs’ RIVM websites.
VECOZO
This system is used to communicate with health insurance companies.
PRAEVENTIS
The data surrounding the 12-week blood test and heel prick are shared with RIVM through this system.
SCIENTIFIC STUDIES
We do not share data with scientific studies without consent. We ask permission from those involved before sharing information about, for example, pregnancy.
PREGNANT-PORTAL
Through Zwangerenportaal.nl, the midwife practice offers clients the opportunity to have digital access to her own medical records, receive additional relevant information around pregnancy and birth. All this is done in an online secure environment using account verification. You also get to see ads from which the pregnancy portal benefits commercially. Based on your zip code and IP address show his ads, this data makes the person traceable. Check Pregnancy Portal’s privacy statement for more information.
BIRTH CARDS
Sent birth announcements and cards are hung in the public waiting area of the practice and subsequently destroyed. If you do not want your name and address information to be visible in the waiting room, please remove it yourself with, for example, a waterproof felt-tip pen.
(ON-LINE) REGISTRATION AND APPOINTMENT
If you register with us on-line or make an appointment with us, the information you provide will be used solely to manage your registration and appointment. Your personal data is processed to complete your registration. The legal basis for this processing is that this data processing is necessary for the legitimate interest of Verloskundigenpraktijk Weesp and of you. Specifically, this interest consists of the interest of Midwifery Practice and you in being able to handle questions, complaints or other types of communication requested by you. The medical record is kept for 20 years according to legal provision of the Medical Treatment Agreement Act. Other personal data provided by you, for example for participation in information evenings, will be destroyed after use. Sent birth announcements and cards are hung in the public waiting area of the practice and subsequently destroyed.

In the event that we have activated the Matomo Analytics tool, then (first party) cookies are set to track visitor statistics. It allows us to map how visitors use our website (how long you stay on a page, from which page you came here and to count the number of unique visitors). All data remains on our server and is never shared with other parties.
SECURITY
Verloskundigenpraktijk Weesp has taken appropriate technical and organizational measures to ensure a risk-appropriate level of security.
